Occlusion of the anterior spinal artery (or in one of the vertebral arteries supplying the ASA) at the level of the medulla oblongata is responsible for the ischemic damage in medial medullary syndrome. Damage to the ASA at more caudal levels of the spinal cord leads to a different clinical presentation, called anterior spinal artery syndrome
