Primary sclerosing cholangitis presents with inflammation, fibrosis, & stricture of intrahepatic & extrahepatic bile ducts. On histology: fibrous obliteration of bile ducts & periductal concentric connective tissue deposition (e.g. Onion-skin pattern). Manifestations: Cholestasis & include jaundice, pruritus, fatigue & cholestatic pattern of liver injury (i.e., elevated alkaline phosphatase, direct hyperbilirubinemia)
