Normally, PaCO2 is the major driver for stimulating central chemoreceptors, but in COPD pts with CO2 retention, sensitivity to CO2 is lost, and hypoxia (O2) becomes the main driver. Peripheral chemoreceptors (carotid bodies) which are responsible for sending PaO2 are suppressed with O2 therapy.
