This patient presents with ascites, jaundice, spider angiomas, palmar erythema, caput medusae, gynecomastia, and hard and irregular liver on palpation. Ultrasound shows disrupted architecture of the liver. These are all signs and symptoms of decompensated chronic liver failure. The patient also has a previous history of intravenous drug use. This presentation is consistent with liver cirrhosis due to chronic infection with hepatitis C virus (HCV).
