Proteinuria, hematuria, and hypertension two weeks after a bacterial pharyngitis, most likely has PSGN. PSGN occurs in children 1-3 weeks after streptococcal pharyngitis or 3-6 weeks following a group A streptococcal (GAS) infection of the skin»nephritic syndrome (characterized by the triad of oliguric proteinuria, hematuria, and hypertension). On LM, the glomeruli are enlarged and hypercellular. Electron microscopy shows dense subepithelial humps on the glomerular basement membrane caused by immune complex deposits.
