AAT deficiency -> increased elastase activity -> loss of elastic fibers and increased lung compliance -> panacinar emphysema. Elevated ALT and AST due to accumulation of dysfunctional AAT in hepatocytes where it is produced -> liver damage and eventually cirrhosis
