Suspect child abuse when the injury and the story of the injury do not match; spiral fractures do not generally occur with a simple fall down the stairs. Other signs of child abuse are retinal hemorrhage and detachment due to shaking, abdominal trauma, scattered circular burns (cigarette burns), and subdural hematomas.
