Anemia of chronic disease is caused by sequestration of iron by the immune system. While anemia of chronic disease is actually the most common form of anemia in SLE, this patient’s peripheral blood smear showing spherocytes in the setting of an increased reticulocyte count suggests a hemolytic process.
