In patients with sickle cell anemia, hydroxyurea has been shown to increase fetal hemoglobin production in place of hemoglobin S, which reduces the chance of RBC sickling and sickling-related complications. It also reduces levels of circulating leukocytes, decreasing the adherence of neutrophils to the vascular endothelium, which reduces the incidence of pain crises and episodes of acute chest syndrome.
