First Line:
- Mu Opioid Antagonist –> Naltrexone –> Inhibit Reward/Reinforce
- Can be initiated while patient is drinking
- decrease alcohol cravings
- Cannot give in Liver disease (metabolized)
- Acamprosate
- glutamate modulator
- Can be used in Liver disease (kidney secrets)
Second Line:
- Aldehyde dehydrogenase inhibitors –> Unable to metabolise ethanol, exaggerates discomfort
- Disulphram: considered second line –> need to be highly motivated –>
